ASSUMPTION OF RISK & LIABILITY WAIVER

Assumption of Risk & Liability Waiver

Please read carefully before enrolling. This is a legally binding document.

I, the undersigned parent or legal guardian, understand and acknowledge that participation in swimming lessons and all related aquatic activities involves inherent risks, including but not limited to slipping, falling, physical injury, illness, paralysis, drowning, or death.

I understand that these risks may arise from known or unknown causes, including instructor actions, participant behavior, pool conditions, or other circumstances.

By enrolling my child/children in Swim Shine Aquatics programs, I voluntarily assume all risks associated with participation.

Release of Liability

I hereby release, waive, discharge, and hold harmless Swim Shine Aquatics, its owner(s), instructors, employees, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may occur during participation in swim lessons, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Authorization

I authorize Swim Shine Aquatics staff to obtain emergency medical treatment for my child in the event it is deemed necessary. I accept full responsibility for any medical expenses incurred.

Supervision Acknowledgment

I understand that instructor attention is focused on enrolled participants and that I am responsible for supervising any additional children or observers present during lessons.

Voluntary Participation

I confirm that participation is voluntary, and I am responsible for determining whether my child is physically and emotionally fit to participate in swim instruction.
